Both have quite a lot of impact if you're wealthy because they're combined with resources and time to do them.
If you're not wealthy you can do them, but you have to sacrifice other things, and some of those you wont be able to afford.
Those services aid will power instead of health directly.
If you are wealthy, it does not matter if they are expensive. If they have any impact, why not?
