The incentives align well -- people get healthcare for free and and healthcare service provider gets a nice, fat government check.
That's why we don't do preventive care -- our current sick care model is much more profitable.

It's not healthcare mostly, it's medical care.
Otherwise we would tax added sugar out to wazoo and use it to pay gym memberships.
There are a lot of misaligned incentives in the US healthcare system, but Medicare and Medicaid health plans generally offer similar preventive care benefits as commercial plans.
